Trained community supporters who help vulnerable citizens onboard, use HealthCentreApp safely, and stay connected to care support.
What CdHPs do (coming soon)
- Help citizens onboard and set up their digital health profile.
- Support safe use of key features (CareRecord, CareRequest, CareConnect, CareDiary).
- Encourage timely escalation to local care services when needed.
- Provide ongoing digital support for up to 50 vulnerable citizens (programme rule).
Eligibility & safeguarding
- Designed for Premium users who complete CdHP training and verification.
- CdHP support is guidance and digital help - not a replacement for clinical diagnosis or treatment.
- Citizen consent governs any access to records and communications.
